Making sure you have enough income to support your family if you become physically challenged and can’t work for an extended period or in the event of your death is an important part of any financial plan for you and your families.

Midwestern offers Basic Life and Accidental Death and Dismemberment Insurance (AD&D) for all benefit-eligible employees. Employees also have the option to purchase Supplemental Life Insurance coverage for themselves, spouse or child(ren).

In order to help provide income in the event of disability, Midwestern offers benefit-eligible employees group short and long-term disability coverage as a benefit. See below for plan specifics and applicable forms:

Basic Life

Life and AD&D insurance is available to all benefit eligible employees. You are automatically covered at two times your annual salary at no cost to you. The maximum Basic Life Insurance is $1,200,000.

Supplemental Life

During your initial enrollment (new hire or status change) you may elect additional life insurance, on yourself, Spouse and/or Child(ren) without providing a Statement of Health form.

Short-Term Disability (STD)

Short-Term Disability (STD) may provide income protection for an employee who is unable to work for an extended period of time. STD is available to employees who are .5 FTE or greater and after six months of employment. It is provided at no cost to the employee.

Long-Term Disability (LTD)

Employees are eligible for the basic Long-Term Disability (LTD) policy after one year of service provided at no cost to the employee. The LTD benefit is 60% of your annual salary up to a maximum of $20,000 per month.

Supplemental Long-Term Disability

During your first year of employment, you may elect to enroll in Supplemental LTD protection at a nominal cost to you for disabilities lasting longer than six (6) months and prior to becoming eligible for the employer-paid Long Term Disability Plan. The plan is voluntary and the premiums are paid 100% by you.
